There are two ways that claims can be submitted to Delta Dental in order to pay for an assigned benefit or dental consultation, treatment or procedure. We’ve compiled a quick guide on how to submit claims to Delta Dental if you are an individual member or receive dental insurance through your employer:

With an In-Network Dentist

Delta Dental uses a network of dentists that are contracted to provide their services. When using a dentist that is considered part of this network, you will not have to pay the bill or claim from your insurance. Your dentist will submit the claim on your behalf to Delta Dental and it will be paid directly by Delta Dental. In this case, it is recommended to use only dentists that part of the Delta Dental network to get the greatest benefit from your insurance.

With Out-Of-Network Dentists

If you choose to use a dentist who is not part of the network, you will be required to complete a claim form. This form can be downloaded and printed from the Delta Dental website. Be aware that there are two forms – one for individual members and one for members who are part of an employer group.

If you are part of an employer group, you should be able to get the form from your HR department. Make sure that you complete the correct form. Once you have completed the form it should be mailed to:

Individual claims should be addressed to the attention of DDIL – Individual Claims with the electronic claims payor ID: IDIND.

Group claims should be addressed to the attention of DDIL – Group Claims with the electronic claims payor ID: ID05030.

It is important that the claim is addressed correctly in order to speed up processing.

If you are having trouble completing the form, call the Delta Dental Customer Service line for assistance. Alternatively, take your claim form with you to your dentist and ask them for assistance completing the form. Remember that the form should be completed in full for efficient processing.

Delta Dental will assess the validity of the claim and whether the services provided are covered by your specific dental policy. They may contact your dentist for additional information regarding services rendered. In general, claims should be processed within 3 working days (does not include week-days or holidays).

You will be reimbursed for the amount of the claim directly once the claim has been approved by Delta Dental. If you have not already paid your dentist, use the reimbursement to pay the dental bill.
